Mail in the following to Dynon Avionics within three months of your D2 purchase.

• Completed Rebate Form: After submitting the rebate form, you will receive a confirmation email containing the information you just submitted. Include a printed copy of either the confirmation email or the pdf attachment.
• Proof of Purchase: A copy of your receipt or invoice from Dynon Avionics or an authorized dealer.
• Proof of your FATPNW Premium Membership: A copy or picture of your FATPNW Premiums membership card.

Rebate valid for the original end-user customer of the D2 only.Dynon dealers, their employees, and employees of Dynon Avionics are not eligible for this rebate. Rebate submissions that do not meet the criteria on this form will be rejected. All rebate submissions must be physically mailed to Dynon Avionics. Email and fax submissions are not valid submission methods. Please allow 4-6 weeks for processing. Dynon reserves the right to clarify and modify this offer at any time.
